Claim Forms
List claim form definitions (e.g. ADA 2006, UB04). Used when creating claims and insurance plans. Per OpenDental API ClaimForms.
What are Claim Forms?
Claim forms define the form type used for insurance claims (e.g. ADA 2006, ADA 2012, UB04). Each has a ClaimFormNum, Description, and IsHidden. Use ClaimForm (or ClaimFormNum) when creating or updating Claims and when setting ClaimFormNum on Insurance Plans. This API is read-only (GET only)—claim forms are managed in Open Dental Setup.
